SUMMARY OF JOB:

Provides personal care and assistance to Home Care and Hospice clients under the supervision of the Case Manager and Clinical Manager

CONNECTING TO MISSION: Holy Redeemer Home Care and Hospice's ability to continue the rich heritage of the Sisters of the Redeemer and care, comfort, and heal in the home is directly related to the talents, motivation and accomplishments of our employees. A primary way staff connect to our mission is by collaborating with all team members in promoting an environment that provides quality, compassionate, holistic, patient-centered care and supports all internal and external customers

RECRUITMENT REQUIREMENTS: High School diploma or equivalent preferred; Must be able to read and understand written instructions; Must be able to take direction via telephone in patient's home; Must be able to make meaningful observations and report changes in patient condition; Must have basic knowledge of computer applications and ability to enter patient information into an electronic device. Successfully pass all competency exams.

LICENSE AND REGULATORY REQUIREMENTS: Qualified for employment under current state (Pennsylvania or New Jersey) state regulations, current driver's license and access to a car, current car registration, and current car insurance.
